RIGHT OF ENTRY AGREEMENT AND LOCATION INFORMATION

RIGHT OF ENTRY AGREEMENT

This Right of Entry Agreement (“Agreement”) is made and entered into by and between the undersigned property owner or authorized representative (“Owner”) and Lummi Nation Connect. The purpose of this Agreement is to allow the Lummi Nation Connect and its authorized contractors to access the property for the installation, maintenance, and operation of fiber optic broadband infrastructure.

The Right of Entry Form gives Lummi Nation Connect permission to access your property so we can install the new fiber‑optic broadband service line. This access is temporary and only allows our team to bring the fiber connection from the road to your home or building.

Here’s what the form covers in simple terms:

• • Basic Property Information: Your address and contact info so we know exactly where the work will take place.

• • Permission to Access: You are granting our technicians permission to enter your property to install and connect the fiber line and related equipment.

• • What We’ll Do: Work may include placing cable, conduit, and any necessary telecom equipment to bring service to your home.

• • Restoring Your Property: When we’re done, we will restore the work area as close as possible to its original condition.

• • No Ownership Changes: Signing the form does not give Lummi Nation Connect any ownership of your property—just temporary permission to complete installation.

• • • Company Responsibility: We are responsible for any damages caused by our work.

   

***Signature from the Owner or their Authorized Representative will be required to complete this work, EXCEPT for Housing Authority properties which do not need to submit an ROE.  The signature form can be downloaded from the link below, or picked up from the front desk of the LIBC Admin building, or signed with a technician on Day of Install.  If completed before Day of Install please sign and return to the front desk of the LIBC Admin building, or scan and email to Broadband@lummi-nsn.gov
